Job summary

Spectrum Retirement Communities is seeking a part-time Wellness Nurse to support residents in Burr Ridge, IL, with 2-10pm shifts and every other weekend. You will collaborate with community nurses, caregivers, and medication staff to ensure regulatory compliance and clinical excellence.

Responsibilities include coordinating admissions, delivering care per care plans, administering meds, coordinating with external providers, documenting care, and staying current with state regulations.

Qualifications

• Knowledge of state regulations, policies and procedures for compliance.
• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing preferred.

• Assist in resident admission process including screening, assessment and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments per physician orders.
• Coordinate care with outside providers and resident physicians.
• Document and report care provided and resident changes.
• Maintain knowledge of state regulations, policies and procedures.

Job Description

Schedule: Part Time 2-10pm and every other weekend

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures

Qualifications

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing, preferred
