Job summary

Oakmont Management Group is seeking a part-time Wellness Nurse at Oakmont of Concord (Concord, CA). The role coordinates resident care and medications under physician supervision, ensuring residents remain as independent as possible.

The position requires a CA nursing license (RN/LVN), CPR/First Aid certification, and ability to work with seniors. Prior RCFE experience preferred; training provided. Full benefits available to eligible staff.

Qualifications

• Must hold a valid California RN or LVN license in good standing.
• Prefer 1 year of experience in Residential Care Facility for the Elderly (RCFE).
• Proficient in MS Office (Word, Excel).
• Able to operate standard office equipment (fax, copier, phone).
• Knowledge of assisted living and dementia care.
• Able to engage with seniors and cognitively impaired individuals.
• Must obtain/maintain valid CPR and First Aid certification.
• Criminal background check and health screening required.

Responsibilities

• Coordinate resident care and medication management under physician supervision.
• Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
• Administer medications and perform treatments as ordered by a physician.
• Support Medication Technicians in medication delivery.
• Assist with resident assessments as directed by Health Services Director.
• Observe residents, report changes to physicians and executives as needed.
• Document thoroughly in resident charts and ensure regulatory compliance.
• Monitor shift reports and communicate pertinent information during handover.
