Oakmont Management Group is seeking a Wellness Nurse to coordinate resident care and medication management at a California senior living community. The role ensures residents stay as independent as possible and provides coverage seven days a week under medical direction.

Responsibilities include collaborating with physicians, coordinating ancillary care, administering medications, supporting Medication Technicians, and documenting thoroughly.

Qualifications

• Must hold a California LVN or RN license in good standing.
• Dementia care experience preferred.
• Experience in RCFE preferred.
• Proficient with MS Office and able to use standard office equipment.

Responsibilities

• Coordinate care with community healthcare professionals.
• Coordinate incidental medical, dental, vision, hearing, and podiatry care.
• Administer medications and treatments as ordered by a physician.
• Assist Medication Technicians to ensure proper medication delivery.
• Assist with resident assessment tasks as directed.
• Conduct staff interviews and communicate resident health changes.
• Document thoroughly in resident charts per policy and regulations.
• Coordinate prescription orders with doctors and pharmacies.
• Communicate with residents regarding preferred pharmacy and incontinence programs.
