Job summary

Oakmont Management Group seeks a Wellness Nurse to coordinate resident care and medication management under physician supervision. The role requires a CA-licensed LVN/RN and the ability to provide consistent coverage seven days a week, including days and evenings.

Qualifications

• Licensed LVN or RN with valid California license in good standing.
• Preferred 1 year experience in a Residential Care Facility for the Elderly (RCFE).
• Proficient with MS Office tools (Word, Excel).
• Able to operate standard office equipment and perform documentation as required.

Responsibilities

• Coordinate resident care and medications under medical direction.
• Ensure consistent coverage seven days a week.
• Communicate with physicians, discharge planners, and staff about resident needs.
• Oversee medication delivery and central storage.
• Complete documentation in resident charts per policy and state regulations.
• Assist with staff interviews and resident health observations.
• Coordinate prescriptions with doctors and pharmacies.
