Job summary

Monarch Communities is seeking a Wellness Nurse (LPN) to provide direct nursing care to residents under the supervision of the Health and Wellness Director. This PRN role emphasizes holistic wellness and supervision of care staff.

The position requires an active LPN license and at least 2 years of experience in long-term care or related settings, with proficiency in MS Office and strong regulatory knowledge. Willingness to lift up to 50 lbs and perform various tasks is expected.

Qualifications

• Graduation from an accredited School of Nursing with current LPN license.
• At least 2 years’ experience in long-term care, assisted living, home health or hospital setting.
• Experience working with residents with Alzheimer’s or other related dementias.
• Ability to handle multiple tasks.
• Knowledge of federal and state regulations, nursing practices and methods.
• Proficiency in Microsoft Office suite.

Responsibilities

• Monitor the health, safety, and well-being of all residents.
• Assist in training and monitoring of medication administration.
• Supervise care staff in accordance with current state regulations and community policies.
• Maintain clinical quality assurance per standards.
• Promote the highest degree of service to residents while aligning with company mission.
• Conduct thorough resident assessments.
