Job summary

Monarch Communities is seeking a Wellness Nurse (LPN) to provide direct nursing care to residents and supervise day-to-day nursing activities; the role reports to the Health & Wellness Director. The ideal candidate holds an LPN license with 2+ years in long-term care or related settings, and experience with Alzheimer's care.

Strong multitasking and MS Office skills are expected, with a weekend-focused schedule under the Weekend Warrior program.

Qualifications

• Graduation from an accredited School of Nursing with current license.
• At least 2 years’ experience in long-term care, assisted living, home health or hospital setting.
• Experience with residents with Alzheimer’s or related dementias.
• Ability to handle multiple tasks.
• Knowledge of federal and state regulations and nursing practices.
• Proficiency in Microsoft Office (Windows, Outlook, Excel).
• Physical ability to lift up to 50 lbs and perform required tasks.

Responsibilities

• Provide direct nursing care to residents and supervise day-to-day nursing activities.
• Supervise care staff in accordance with regulations and policies.
• Monitor health, safety, and well-being of residents.
• Assist in training and monitoring medication administration.
• Maintain clinical quality assurance per standards.
• Lead and model the mission of Monarch Communities.
• Attend in-service trainings and ensure compliance with regulations.
• Perform other duties as assigned.
