Job summary

Civitas Senior Living in Fort Worth, TX is seeking a Wellness Director / Director of Nursing to lead care teams and drive person-centered care for residents. You will oversee assessments, care planning, and coordination with physicians, vendors, and staff to ensure safety and quality across all services.

The role emphasizes leadership excellence, regulatory compliance, and continuous oversight of medication management and audits, with a focus on preserving resident independence and wellbeing.

Qualifications

• Minimum 2 years as a Wellness Director or Director of Nursing.
• Active LVN/LPN or RN license in the state.
• Strong customer service and hospitality mindset.

Responsibilities

• Lead a full care team through recruiting, interviewing, mentoring, and coaching.
• Perform comprehensive assessments, coordinate individualized care plans, and provide empathetic, personalized care.
• Liaise with internal staff, third-party vendors, and medical professionals.
• Lead staff meetings, manage service agreements, update physician orders, and assure compliance with healthcare protocols.
• Ensure proper medication management, complete timely audits, handle incident reporting, and conduct investigations.
