Civitas Senior Living in Winnsboro, TX, invites you to join Autumn Wind as Wellness Director / Director of Nursing. Lead a dedicated team to deliver resident-centered care, develop care plans, and ensure compliance with health regulations.

You will collaborate with staff and vendors, oversee medication management, and drive quality improvements. An active TX RN/LVN/LPN license and at least 2 years of leadership experience are required. We offer a supportive community and generous paid time off.

Qualifications

• Experience: Minimum 2 years as a Wellness Director, Director of Nursing, or equivalent, preferably in senior care.
• Licensure: Active LVN/LPN or RN license in the state of Texas.
• Professionalism: Stellar customer service and hospitality skills.

Responsibilities

• Lead a full team through recruiting, interviewing, mentoring, and coaching.
• Perform comprehensive assessments, coordinate individualized care plans, and provide empathetic, personalized care.
• Liaise with internal staff, third-party vendors, and medical professionals.
• Lead staff meetings, manage service agreements, update physician orders, and ensure regulatory compliance.
• Ensure proper medication management, complete timely audits, and handle incident reporting.
• Undertake other duties aligned with Civitas' mission and vision.
