Job summary

Sycamore Rehabilitation Services, Inc. is seeking an LPN to deliver Wellness Coordination services, monitoring health status, high-risk plans and medication administration for persons served.

The role includes training staff on Medication Administration, High Risk Plans, and Wellness Plans, and collaborating with the IST and health professionals to identify health needs and strategies to update wellness plans.

Qualifications

• Excellent verbal and written communication skills.
• Ability to communicate effectively, make presentations and write concise reports for all levels of employees.
• Dependable transportation, valid Indiana driver's license, and proof of personal automobile insurance.

Responsibilities

• Provide Wellness Coordination services for designated individuals per plan documentation.
• Conduct Medication Administration and high-risk plan oversight, and coordinate with health professionals.
• Provide face-to-face visits and other approved methods of Wellness Service provision and document accordingly.
• Coordinate trainings on Wellness Coordination plans and in-service sessions for staff.
• Develop and maintain High-Risk Plans and audit Wellness plans as required.
