Job summary

Sycamore Services Inc in Evansville, Indiana, seeks a Licensed Practical Nurse (LPN) to deliver Wellness Coordination. The LPN will train staff on Medication Administration, High Risk Plans, and Wellness plans for persons served.

Responsibilities include Wellness coordination for designated individuals, development of High‑Risk Plans, and consultation with IST and other health professionals to update wellness plans.

Qualifications

• Must possess a current Indiana nursing license, LPN.
• Two or more years’ experience working with individuals with disabilities preferred.
• Previous supervisory and/or training experience preferred.
• Excellent verbal and written communication skills.

Responsibilities

• Provide training on Wellness Coordination plans to new hires during orientation sessions and as needed.
• Coordinate in-service training to meet persons served needs in conjunction with Wellness and High-risk plans.
• Conduct Medication Administration Competency testing during orientation and recertification.
• Provide Wellness coordination for designated individuals per plans and policies.
• Develop and maintain High-Risk Plans for participants.
• Consult with IST and health professionals to update wellness plans.
• Face-to-face visits and other approved methods with proper documentation.
• Ensure medication policies and procedures are followed and train staff accordingly.
• Complete audits and prepare reports with recommendations.
