Job summary

Morrison Healthcare, a Compass Healthcare company, seeks a Community Support Specialist to support Prisma Health and underserved teammates in surrounding communities. The role will coordinate housing, food, legal services, mental health resources, ensuring a safe environment.

Responsibilities include needs assessments, using LiveWell to connect teammates with resources, assisting with benefit enrollment, liaising with the emergency fund, and building community partnerships.

Qualifications

• Bachelor’s degree preferred in Health Education or related field.
• MSW or Certified Health Coach preferred.
• Two years’ healthcare experience required or preferred.

Responsibilities

• Conduct and analyze needs assessment data on social determinants of health.
• Utilize LiveWell platform to connect teammates with resources.
• Assist teammates with applications for assistance and benefit enrollment.
• Liaise with emergency care fund to obtain financial approval when needed.
• Build relationships with community partners to secure resources.
• Communicate with department managers to ensure proper support.
• Educate teammates on whole-person health concepts.
• Follow up at 6, 12, and 24 months to monitor progress.
