Job summary

Services for the UnderServed (S:US) seeks a Wellness Coach in New York to assist agency consumers in accessing medical, mental health, substance use services and community resources. The role includes developing a referral network, case management, and follow-up to support community integration.

The position emphasizes advocacy, crisis intervention, and collaboration with therapeutic services. On-site work with a focus on housing-related supports and patient advocacy is required.

Qualifications

• Bachelor's Degree preferred, not required or 2 years of experience with behavioral health population.
• Working knowledge of e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System.

Responsibilities

• Provide case management to assigned caseload.
• Develop and monitor individual support plans in collaboration with agency consumers.
• Assist agency consumers with problem solving and crisis intervention.
• Accompany agency consumers on medical, psychiatric, and public benefit appointments.
• Act as advocate for consumer; encourage and support self-advocacy.
• Identify problems in daily living skills; provide hands-on assistance or referrals.
• Support Facilities Management staff to maintain apartment standards per S:US and DOHMH.
• Provide supportive counseling to agency consumers as needed.
• Conduct regular home and hospital visits as necessary.
• Document all services per agency standards.
