Responsibilities

  • Provide case management to assigned caseload.

  • Assist agency consumers with problem solving and crisis intervention.
  • Accompany agency consumers to medical, psychiatric, and public benefit appointments.
  • Advocate for consumers and support self-advocacy.
  • Identify problems in daily living skills and provide hands-on assistance or referral.
  • Maintain strong working relationships with therapeutic support services.
  • Document all services in compliance with agency standards.
  • Support moves, lifting, packing and cleaning as needed.
  • Work with the social service team to develop off-site activities for community inclusion.
  • Liaise with community services and groups; participate in supervision and meetings.

Position Overview

I. SCOPE OF ROLE:The Rehabilitation Specialist engages people with mental illness to assist them in meeting the demands of community living and maintaining their mental health by teaching them the necessary skills, and linking them to services that will support their recovery, independence, entitlement, and self determination.


II. ESSENTIAL DUTIES & RESPONSIBILITIES:



  • Provide case management to assigned caseload.

  • Develop and monitor individual service plans in conjunction with agency consumers.

  • Assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up.

  • Assist agency consumers with problem solving and crisis intervention.

  • Accompany agency consumers on medical, psychiatric, and public benefit appointments, as necessary.

  • Act as advocate for consumer as appropriate; encourage and support self- advocacy.

  • Identification of any problems in daily living skills; hands on assistance and/or referral in addressing problems in daily living skills.

  • Maintenance of a strong working relationship with all therapeutic support services to ensure the best quality of care.

  • Documentation of all services in compliance with agency standards

  • Assist with moves with consumers and staff; also include lifting, packing and cleaning

  • Work as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integration.

  • Liaison to community-based services and groups including civic, social service government, and business organizations.

  • Participate in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainings

  • Adhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate action.

  • Other duties as requested by supervisor

III. REQUIREMENTSREQUIRED EDUCATION AND EXPERIENCE



  • Knowledge of Microsoft Office tools

  • Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibility


PREFERRED QUALIFICATIONS & SKILLS



  • High School Diploma or equivalent preferred.

  • Valid driver’s license preferred.

  • Experience working with people who have a mental illness preferred.

  • Basic knowledge of human services.

  • Bilingual (Spanish/English) speaking preferred
