Job summary

WellLife Network in New York, NY seeks a Family/Housing Specialist to provide intake assessments, develop service plans, and coordinate services for tenants in residence, focusing on the strengths and needs of families including children.

You will maintain case notes, monitor progress, and connect tenants with community resources while participating in meetings and trainings; a valid NY driver’s license and ability to travel are required. A bachelor’s degree with relevant experience is preferred.

Qualifications

• Bachelor's Degree with 1 year of work with homeless, families with children, or mentally ill populations.
• Valid New York State driver’s license.
• Experience supporting tenants in housing or related services is preferred.

Responsibilities

• Provide intake assessments and develop service plans.
• Coordinate services and record case notes per agency standards.
• Monitor tenant progress and maintain tenant files monthly.
• Assist with evacuation planning and safety in emergencies.
• Attend meetings and trainings at various sites; transport as needed.
• Support tenants in recreational and social activities and community integration.
