Qualifications

• BA/BS degree from a four-year college/university required.
• Two years of direct service experience working or volunteering in a human service organization.
• Ability to obtain Release of Information for care coordination.

• Coordinate client care with internal/external providers.
• Provide life skills training, care management, and structured interventions.
• Assist clients in accessing resources and self-management of symptoms.
