Qualifications

• Minimum 3 years of related healthcare coordination experience.
• Proficient in computer applications and typing skills.
• Bilingual preferred.
• Experience with Electronic Medical Record systems preferred.

Responsibilities

• Coordinate care management and ensure timely consultations.
• Educate patients about diagnoses, treatments, and community resources.
• Assist with chronic disease education and goal setting with patients.
• Collaborate with primary care providers to maximize benefits of care.
