Job summary

Centerstone in Indiana is seeking a Care Coordinator to support Behavior & Primary Healthcare Coordination (BPHC)-enrolled clients. You will provide individualized coaching to improve health outcomes and coordinate care with a team of providers, helping clients access necessary resources and improve self-management of symptoms.

The role involves health hygiene, lifestyle guidance, nutrition, and navigation of healthcare systems within the CC BCHC framework.

Qualifications

High School diploma or GED required; BA/BS preferred.
Two years of direct service experience in human services.
Ability to coordinate with a multidisciplinary team and external providers.

Responsibilities

Provide individual, supportive coaching to improve physical health status for assigned clients.
Coordinate client care with a team of internal/external providers and obtain Release of Information as needed.
Deliver life skills training, care management, and structured interventions to improve access to resources.
Assist clients in scheduling appointments and navigating healthcare services.
