Provides individual, supportive coaching to improve physical health status; coordinates client care with team of internal/external providers; aids clients in accessing/utilizing resources; and promotes the best outcomes for our clients.ESSENTIAL DUTIES & RESPONSIBILITIESProvides individual, supportive coaching to improve physical health status for assigned clients, such as help in scheduling appointments with doctors, coaching on communicating more effectively with doctors and following instructions on medications or other doctor recommendations.Coordinates client care with team of internal/external providers. Obtains Release of Information for all medical providers for coordination of care.Provides life skills training, care management, and structured interventions to assist consumers in gaining access to resources and the skills necessary to improve their physical health status, quality of life, and self-management of symptoms.
