Job summary

Weight Loss & Wellness Clinic in Lake Forest, IL, is seeking a Nurse Practitioner/Physician Assistant for a part-time, on-site role. You will evaluate patients, plan treatments, and educate patients under established protocols. Collaboration with a supportive team is emphasized.

Qualifications

• Active Nurse Practitioner license in good standing.
• Board certification (AANP or ANCC).
• Ability to work independently within established protocols.
• Strong communication and interpersonal skills.
• Comfortable learning new systems and treatment models.
• Additional Certifications.

Responsibilities

• Conduct patient evaluations, including medical histories and progress visits.
• Develop and manage individualized weight loss treatment plans under protocols.
• Prescribe medications and therapies according to state laws.
• Monitor patient progress, labs, and outcomes.
• Educate patients on weight loss, nutrition and lifestyle changes.
• Document accurately in the EMR/EHR system.
• Collaborate with medical staff and clinic leadership.
• Maintain HIPAA compliance and clinic policies.
• Support a compassionate patient experience.
