Job summary

The Groves in Independence, MO is seeking a Weekend Wound Care Nurse (RN or LVN) to join our skilled nursing facility clinical team. This role is responsible for assessing, treating, documenting, and monitoring all wounds within the facility while working collaboratively with the interdisciplinary team to ensure quality care.

Qualifications

• Current and valid RN or LVN/LPN license in MO.
• Minimum 1 year of wound care experience in SNF or long-term care.
• Wound Care Certification (WCC/WOCN) preferred but not required.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Familiarity with EMR systems (PointClickCare) is a plus.

Responsibilities

• Conduct wound assessments and provide care per orders and protocol.
• Monitor healing progress and adjust care plans as needed.
• Perform dressing changes, debridement if licensed, and pressure injury prevention.
• Document wound care and progress in the EMR accurately.
• Communicate with physicians and care team about wound status.
• Educate staff on wound care techniques and prevention.
• Participate in wound care rounds and QAPI meetings.
