Job summary

Advocate Health in Chicago seeks a licensed Physical Therapist for full-time orthopedics/rehabilitation. The role includes evaluating referrals, creating treatment plans, and coordinating patient discharge within the acute rehab setting.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Maintains CE requirements per state standards.
• Licensed Physical Therapist in the applicable state.

Responsibilities

• Assess referrals and complete musculoskeletal, neuromuscular, cardio-pulmonary evaluations.
• Develop and implement care plans and document outcomes for each patient.
• Coordinate discharge planning and home programs with patients and families.
• Complete documentation including progress notes and discharge summaries.
• Maintain cleanliness and safe operation of treatment areas.
• Support performance improvement and coverage as needed.
• Mentor and supervise PT aides and students when required.
