Chestnut Woods Rehabilitation & Healthcare Center is seeking a Weekend Warrior Nurse (RN/LPN) to join our compassionate team in Massachusetts. This PRN role schedules 5 consecutive weekends with the 6th off, with shifts from 7am to 3pm and daily pay options.

Responsibilities include providing skilled nursing care, administering medications, monitoring residents, documenting accurately, and collaborating with the interdisciplinary team to ensure high-quality, person-centered care.

Qualifikationen

• Current MA RN/LPN license.
• BLS CPR certification required.
• Ability to manage care for multiple residents in a fast-paced setting.
• Strong clinical assessment, communication and documentation skills.

• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments per care plan.
• Monitor residents’ conditions and report changes promptly.

Responsibilities for Nurse (RN/LPN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments in accordance with the plan of care (POC).
• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Nurse (RN/LPN):

• A current, unencumbered active license to practice as an RN/LPN in the state of Massachusetts.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
