Job summary

ABridge Care Concepts, LLC is seeking a compassionate Caregiver to support seniors in our on-site care cottages. You will provide personalized care, assist with transportation, and help residents with daily activities to promote independence and well-being.

The role involves coordinating with therapists, administering prescribed medications, monitoring health needs, and engaging residents in memory care and mobility programs, ensuring safety and dignity at all times.

Qualifications

• High school degree or equivalent.
• Prior caregiving experience.
• Availability to work night shifts and weekends.

Responsibilities

• Providing personalized care and assistance to residents.
• Assisting with transportation arrangements and making appointments.
• Engaging residents in health promotional activities such as exercise groups and walking clubs.
• Administering prescribed medications and managing medication schedules.
• Communicating with physicians and other healthcare personnel.

• Assisting with ambulation and mobility programs.

• 24 hour, awake staff on duty.

• Basic and moderately advanced Health services included in one (not all) of assessed levels (see fee schedule) may be:
• Monthly weights and vital signs
• Limited, prescribed therapeutic diets (pureed, mechanical soft, thickened liquids)
• Glucose monitoring, insulin administration
• Medication management
• Communication with physicians and other designated health care personnel
• Memory Care and Dementia Related Behavioral interventions
• Functional Maintenance Programs as developed by therapists
• Ambulation and mobility programs
• Monitoring of identified personal needs of bathing, dressing, hygiene
