Job summary

Qualifications

• Graduate of an accredited nursing program.

• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities; ability to supervise, lead, and mentor nursing staff.

Responsibilities

• Provide direct clinical care and documentation for residents.
• Administer medications and maintain accurate records.
• Perform daily rounds and resident assessments.
• Communicate changes in condition to residents, families, and providers.
• Develop and update resident care plans.
• Supervise nursing staff and train new personnel.
• Ensure safety, OSHA compliance, and infection control.
• Participate in quality assurance and state surveys.

Position Summary

Bradford Place Care Center is a full-service nursing facility specializing in complex care. We are committed to providing high-quality clinical and rehabilitative services in a compassionate, supportive environment designed to meet the evolving needs of our patients and residents.

LPNs are responsible for providing compassionate, high-quality care to residents while ensuring compliance with all regulatory requirements. This role involves direct clinical care, documentation, communication with interdisciplinary teams, and supervision of nursing staff

• Perform daily rounds and assessments on assigned residents.
• Monitor and ensure delivery of high-quality resident care.
• Administer medications and maintain accurate documentation.
• Communicate changes in condition to residents, families, and healthcare providers.
• Assist in developing and updating resident care plans.
• Support and train nursing staff, including new and temporary employees.
• Ensure safety and compliance with OSHA, infection control, and emergency protocols.
• Participate in quality assurance and state survey processes.

Supervisory Duties

• Direct supervision of nursing staff and indirect supervision of all staff involved in resident care.
• Involved in hiring, training, performance management, and staff scheduling.

Job Requirements:

Education & Experience

• Graduate of an accredited nursing program.
• 1+ year of nursing experience preferred.

Licensure

• Must hold a validLPN Licensein the state of employment.
