The Triage Registered Nurse - Hospice role covers weekends with base pay plus per-visit and per-admission incentives, serving a broad NH service area including Londonderry.

Qualifications

• 2+ years nursing experience, geriatrics preferred.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient needs change.
• Responds to calls during afterhours on weekends (8:30 AM–8:00 PM).
• Assesses patient needs through triaging and in-home care.
• Collaborates with physicians, NPs and interdisciplinary team on Plan of Care.
• Responds to inquiries from patients, caregivers and medical team.
• Consults patients and caregivers before, during and after treatment.
