Job summary

Pathways Healthcare, a hospice provider in Massachusetts, seeks an On Call Hospice Triage Nurse for weekend shifts in Westwood. You will triage calls, initiate care plans, and coordinate home visits as needed.

Join a team that collaborates with physicians and the interdisciplinary staff; requires 2+ years nursing experience (geriatrics preferred), a nursing degree, proficiency with electronic medical records, and a current BLS CPR certification.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
