Job summary

Bon Secours is seeking a Social Worker Care Manager for St. Francis Downtown. This part-time, weekends-only role focuses on discharge planning and coordinating care across hospital, community, and government resources to ensure smooth transitions for patients and families.

You will provide clinical supervision to peers and students, identify patients needing social services, and collaborate with the healthcare team to optimize outcomes. Strong communication and ethics are essential.

Qualifications

• Bachelor of Social Work required.
• Master’s in social work or healthcare field preferred.
• BLS Basic Life Support (American Heart Association) required.
• Licensed Social Worker in state of practice required; VA preferred.
• Accredited Case Manager (ACM) or Certified Case Manager (CCM) preferred.

Responsibilities

• Identifies and prioritizes patients in need of social services using a biopsychosocial approach and coordinates care with the healthcare team.
• Completes documentation reflecting plan of care and transition needs in the patient record.
• Ensures post-hospital care planning and resource access with patient, family, and caregivers.
• Follows standardized practices for Advance Care Planning, Length of Stay, and readmission prevention.
• Supports and promotes proactive care by addressing barriers to progression of care and participating in discharge rounds.
• Promotes timely testing and treatment through collaboration with care team.
