You will process referrals from all sources, work across multiple systems, and may visit patients. An RN licensed in CT is required; on-call support may include non-RN clinicians.

Qualifications

• Registered Nurse licensed to practice in Connecticut.
• Graduate of an accredited School of Nursing.
• Non-RN licensed clinician may apply with RN on call.
• Must be computer literate and able to operate across multiple systems.
• Homecare experience preferred.

Responsibilities

• Ensure Home Care patients receive available services on weekends and holidays.
• Process new referrals from all referral sources.
