Middlesex Health is seeking a RN Triage/Weekend Coordinator for Homecare in New Britain, CT. This part‑time role (16 hours, includes every other weekend) supports patients with weekend services and processes new referrals from all sources.

Responsibilities include coordinating on weekends/holidays in alignment with hospital objectives and policies. Candidates must be licensed as an RN in Connecticut and be comfortable working across multiple systems.

Qualifications

• RN licensed to practice in Connecticut.
• Non-RN licensed clinician may fill with RN administrator on call.
• Computer literate and able to work in multiple systems.
• Must be able to multitask and may visit patients.
• Homecare experience.
• Certification in specialty area preferred.

Responsibilities

• Triage RN/Weekend Coordinator ensures Home Care patients receive services on weekends/holidays per Middlesex Hospital policies.
• Processes new referrals from all referral sources.
