Job summary

Northwestern Medicine is seeking a Registered Nurse for weekend shifts to support patient care across our system. The RN will perform comprehensive assessments, monitor changes in patient conditions, and coordinate discharge planning with the care team.

The role requires an Illinois-licensed RN with BLS certification. The candidate will participate in care coordination, education delivery, and quality initiatives within a supportive, patient-centered environment.

Qualifications

• Registered Nursing license issued by the State of Illinois.
• Bachelors of Science Nursing.
• BLS through American Heart Association.

Responsibilities

• Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation.
• Communicates changes in patient’s condition promptly.
• Utilizes nursing diagnosis and data from the assessment to derive realistic outcomes and documents them as measurable goals on the plan of care.
• Collaborates with patient, family and other health care team members when planning patient care and documents accordingly.
• Assesses patients with regard to discharge planning and makes appropriate referrals as necessary.
• Assures discharge instructions, medication reconciliation and provides information to patient/family regarding the use of patient portal and other resources.
• Demonstrates safe medication administration and participates in unit safety initiatives.
• Attends internal professional development offerings and maintains necessary certifications.
