The RN will perform comprehensive patient assessments, coordinate care with families, document outcomes, support discharge planning, and contribute to unit quality initiatives while maintaining ethical practice and patient safety.

Qualifications

• BSN required or in progress.
• Illinois RN license required.
• BLS certification required.

Responsibilities

• Performs a comprehensive assessment on all assigned patients.
• Communicates changes in patient’s condition promptly.
• Collaborates with patient, family, and health care team in planning care and discharging patients.
• Documents outcomes and updates plan of care as patient conditions evolve.
• Supports discharge planning and referrals as necessary.
• Maintains ethical practices and patient safety in all interactions.
