Qualifications

• Licensed Registered Nurse (RN) in Massachusetts or Licensed Practical Nurse (LPN); Bachelor’s preferred.
• Minimum 2 years of nursing experience and at least 1 year of hospice or equivalent experience.
• Exceptional communication skills.

Responsibilities

• Provides home and hospital visits, conducts evaluations, and coordinates care planning.
• Uses triage and assessment skills to titrate medications.
• Educates patients and families about terminal diseases.

Skills
