Travel to provide care in patients' private homes and residential facilities throughout Greater Boston and parts of MetroWest. The Weekend Nurse is an integral part of an interdisciplinary hospice team responsible for caring for patients at the end of life. Makes home and hospital visits, provides information sessions, evaluates new patients, facilitates admissions to hospice services, and coordinates care planning. Employs triage and assessment skills to titrate medications and manage symptoms appropriately and provides education to patients and families about terminal diseases and end-of-life processes. Communicates with an interdisciplinary hospice team about changes in patients' needs and outcomes of interventions.
Friday, Saturdays, Sundays and Mondays, from 9 am to 7 pm (40 hours/week).
$84,000 - $104,000: This number reflects an average starting range for this position. Individual pay is determined through a variety of factors including experience, education, certifications, and skills and can exceed the range.
