Job summary

Enhabit Home Health & Hospice is seeking a Full-time Weekend RN to provide skilled nursing visits in the home setting. Friday and Saturday shifts are required, with coordination across care disciplines and ongoing communication with physicians.

Responsibilities include accurate OASIS collection, maintaining current medication profiles, and attending weekly case conferences and monthly meetings. Eligible candidates hold an RN license, a valid driver’s license, and preferably 1+ year of clinical

Qualifications

• Current nursing practice knowledge and skills required.
• Live in the employed state (RN license).
• Valid state driver’s license and auto liability insurance.

Responsibilities

• Performs skilled nursing visits and coordinates client care.
• Point of contact for all care disciplines involved with patients.
• Oversees visit frequency for each episode and communicates with physicians.
• Performs accurate OASIS collection and ensures current medication profiles.
• Reports lab values to physicians in a timely manner.
• Attends weekly case conference and monthly case manager meetings.
