Job summary

Harris Home Health, part of LHC Group and a member of the Optum family, seeks an RN for home health. You will provide nursing care at patients' homes and coordinate total care with physicians and the health team.

Responsibilities include developing plans of care, supervising aides and nurses, completing OASIS assessments, and ensuring accurate documentation. A strong emphasis on teamwork and quality outcomes is required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle or public transportation.
• CPR Certification preferred or within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed staff; assigns tasks per regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation.
• Documents patient visits per policy and payer requirements; syncs with policy.
• Participates in on-call and weekend rotation; supports survey readiness.
