Job summary

UnitedHealth Group, supporting Baptist Home Care and LHC Group, is seeking a Registered Nurse in Home Health. You will provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations and agency policies.

You will coordinate the plan of care with other health care professionals, supervise home health aides and RNs, document visits, and participate in on-call rotations to ensure continuous, high‑quality patient care and positive

Qualifications

Current and unrestricted RN licensure in state of practice.
Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
CPR certification preferred or ability to obtain within 90 days.

Responsibilities

Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
Coordinate total plan of care with other health care professionals involved in care and ensure continuity of patient care by planning and exchanging information with physicians and families.
Supervise home health aides and licensed nurses, provide instruction, and assign tasks per state and federal regulations; perform required supervisory visits.
