Qualifications

• Active State Registered Nurse licensure is required.
• Proof of CPR certification.
• Valid Driver's license.
• Minimum 1 year of nursing experience; 2+ years in acute or post-acute care preferred.

Responsibilities

• Manage patient care from admission through discharge per the Plan of Care.
• Collaborate with patient, family, physicians, and care team.
• Submit complete, timely clinical documentation and progress notes.
• Participate in case conferences to review services and goals.
