Part time Registered Nurse Case Manager

Anticipated Territory : Yountville, California

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

Field Requirement: All opportunities at 21HHS require being in the field visiting patients in their homes.
