IU Health is seeking a Registered Nurse Case Manager for a weekend program in Indiana. The role centers on coordinating patient care from admission through discharge, ensuring timely access to services and safe transitions to the next level of care.

The ideal candidate will have 3–5 years of acute care case management experience, an Indiana RN license, and a passion for patient advocacy and resource optimization. Weekend schedule is Friday through Monday with shift variations.

Qualifications

• Active RN license in Indiana or a compact state (NLC) required.
• 3–5 years of nursing experience, inpatient hospital case management preferred.
• Knowledge of Medicare/Medicaid, insurance guidelines and regulatory requirements preferred.
• Certifications in Case Management preferred.

Responsibilities

• Coordinate patient care during hospitalization and discharge planning.
• Assess level of care and resource needs for safe transitions.
• Collaborate with physicians, social workers and care teams to develop care plans.
• Obtain authorizations with insurers and third-party payers.
• Review documentation and regulatory guidelines to ensure compliance.
• Support denial prevention and assist with appeals when needed.
• Educate patients and families about transitions, resources and prevention.
• Ensure adherence to federal, state, payer and organizational requirements.
