Qualifications

• Active RN license in Indiana required.
• Minimum 2+ years as an RN Case Manager.
• Current CPR certification from the American Heart Association.
• Case Management certification preferred.
• Strong assessment, critical thinking and collaboration skills.

Responsibilities

• Coordinate and manage patient care plans across providers and services.
• Assess clinical, psychosocial, and functional needs to identify interventions.
• Collaborate with physicians, nurses, social workers, insurers and community resources.
• Monitor progress, outcomes and barriers; adjust plans as needed.
• Advocate for patients to ensure quality, safe transitions and appropriate resource use.
