Qualifications

• Associates degree from an accredited RN program.
• Current license to practice as registered nurse in the State of Ohio.
• Minimum of 3-5 years recent clinical experience in an acute care setting.

Responsibilities

• Coordinate patient care throughout the ED stay and transition process.
• Collaborate with ED physicians, nurses, social workers, hospitalists, and specialists to ensure safe and efficient care.
• Review admission criteria and medical necessity to support appropriate utilization.
• Develop discharge plans early in the ED stay and arrange post-discharge services.
• Educate patients and families on treatment plans, options, and post-discharge resources.
• Identify barriers to care (transportation, housing, insurance, meds) and coordinate solutions.
• Monitor throughput, length of stay, and discharge disposition outcomes.
