Job summary

Mount Carmel Health System is seeking an experienced RN Case Manager for a part-time, 25-hour-per-week Day shift. The role directs and coordinates patient care from admission through discharge, focusing on cost-effective utilization and patient advocacy within a Catholic health system affiliated with Trinity Health.

Qualifications include an Associate degree in nursing, current Ohio RN license, and 3–5 years of acute care experience. Weekend shifts may be required as needed.

Qualifications

• Associates degree from an accredited RN program.
• Current license to practice as registered nurse in the State of Ohio.
• Minimum of 3-5 years' recent clinical experience in an acute care setting.
• Must be able to work occasional weekend shifts, as needed.

Responsibilities

• Directs and coordinates the care of patients from admission to transfer or discharge.
• Identify discharge needs and determine appropriate level of care.
• Ensure all clinical information is provided to outside agencies and communicate effectively with the care team.
• Eliminate barriers to effective patient stay management and improve processes.
• Maintain licensure and pursue professional development.
• Advocate for patient rights while balancing resources and hospital stewardship.
• Refer quality issues to appropriate sources and coordinate education for patients/families.
