Job summary

ChristianaCare is seeking an RN Case Manager for the Weekend Incentive Program (WIP) in Newark, DE. You will work 12-hour days on Saturdays and Sundays with one rotating Friday shift, 3 out of 4 weekends in a four-week schedule, and an annual rotating holiday schedule.

Qualifications

• Active RN license in DE or a Compact State.
• BSN required.
• 1 year of RN Case Manager experience preferred.
• BLS preferred.
• Case Management Certification within 18 months of eligibility.

Responsibilities

• Identify patients with post-acute care needs and discharge planning.
• Collaborate with physicians, nurses and other health care providers to develop multidisciplinary care plans.
• Lead discharge planning and ensure timely transitions across care continuum.
• Identify barriers to care and participate in strategies to remove them.
