Job summary

ChristianaCare Health System in Newark, DE is seeking an RN Acute Care Case Manager for the Weekend Incentive Program (WIP). You will work 12-hour days on Saturdays and Sundays with a rotating Friday shift and 3 of 4 weekends in a four-week cycle.

The role focuses on discharge planning within an acute care hospital setting, collaborating with the care team to ensure timely transitions and safe care continuums.

Qualifications

• Active RN license in DE or a Compact State.
• BSN required.
• One year of RN Case Manager experience preferred.
• Experience in an inpatient hospital setting with focus on acute care required.
• Case Management Certification (CMC) required within 18 months of eligibility.
• BLS preferred.

Responsibilities

• Identify patients with post-acute care and complex discharge planning needs.
• Collaborate with physicians, nurses and other health care providers to establish discharge plans.
• Review admissions and ensure multidisciplinary care plans are in place to meet patient needs and outcomes.
• Identify barriers to care and participate in strategies to remove them and facilitate patient progression.
