Job summary

ChristianaCare – Newark Hospital is seeking an RN Case Manager for the Weekend Incentive Program (WIP) on the Day Shift. You will coordinate patient care, drive progression, and develop discharge plans within the interdisciplinary team.

Requirements include active DE RN license or compact, BSN, 2 years acute care, and CCM within 18 months. Weekend schedule, 3 of 4 weekends, with additional Friday shift and rotating holidays.

Qualifications

• Active RN license in DE or Compact State is required.
• BSN required.
• Case Management Certification (CCM) required within 18 months of eligibility.
• Minimum of 2 years of Acute Care Hospital setting experience.
• Minimum of 1 year of experience as an Acute Care RN Case Manager or RN Utilization Management.

Responsibilities

• Manage patient care, drive patient progression, and establish a discharge plan.
• Function as a member of the interdisciplinary team and monitor treatment plans for safe, timely transitions.
• Identify barriers to care and participate in strategies to remove barriers and facilitate patient progression.
