Job summary

ChristianaCare in Newark, DE is seeking an RN Case Manager for the Weekend Incentive Program (WIP). The role requires 12-hour day shifts on Saturdays and Sundays with a rotating Friday, in a four-week cycle, plus an annual rotating holiday.

The position offers a special weekend pay rate with full benefits (excluding PTO). A discharge planning focus, collaboration with the care team, and emphasis on safe transitions are central to the role.

Qualifications

• Active DE RN license or Compact State required.
• BSN degree required.
• 1 year of RN Case Manager experience preferred.
• Experience in inpatient hospital setting with acute care focus required.
• Case Management Certification (CMC) required within 18 months of eligibility.
• BLS preferred.

Responsibilities

• Identify patients with post-acute care and discharge planning needs based on comprehensive assessment.
• Anticipate, initiate, and establish discharge plans in collaboration with the care team.
• Review admission assessments and coordinate multidisciplinary care plans.
• Identify barriers to care and help implement strategies to remove them.
