Job summary

ChristianaCare seeks an RN Case Manager for its Weekend Incentive Program (WIP) in Newark, DE. The role requires managing patient care, establishing discharge plans, and collaborating with an interdisciplinary team. Applicants need an active RN license in Delaware, a BSN, and preferably one year of experience as an RN Case Manager. Qualifications

• Active RN license in Delaware or a Compact State.
• BSN required.
• One year of experience as an RN Case Manager preferred.
• Experience in an inpatient hospital setting focused on acute care.
• BLS preferred.
• Case Management Certification (CMC) required within 18 months.

Responsibilities

• Manage patient care and drive patient progression.
• Establish discharge plans and coordinate transitions of care.
• Serve as a member of the interdisciplinary team, creating, implementing, and monitoring treatment plans.
