CP04 Carilion Franklin Memorial Hospital is seeking a Full-Time RN Case Manager to provide case management for assigned patient populations within a dynamic healthcare environment. Responsibilities include conducting patient assessments, facilitating care transitions, and ensuring optimal resource outcomes.

The ideal candidate will have a Bachelor's degree in nursing, Virginia RN licensure, and three years recent experience in a clinical setting. The position comes with a range of benefits including medical, dental, vision coverage, and a pension plan.

Qualifications

• Registered Nurse with a Bachelor’s degree or 5 years RN experience.
• Three years recent experience in a clinical health care setting with direct patient care.
• AHA BLS-HCP certification within 6 months of hire.
