Job summary

AdventHealth in Rome, GA is hiring a Registered Nurse (RN) Care Manager to collaborate with the care team on discharge planning, patient education, and coordination of services to ensure timely transitions. The role requires 2+ years of medical/hospital RN experience, an active RN license, and strong leadership and communication skills.

Schedule is Saturday and Sunday, 10-hour days, with day shifts in a part-time arrangement.

Qualifications

• RN license with active status
• 2+ years RN hospital/medical experience
• Experience in care management or utilization management preferred
• CCM/ ACM certifications preferred
• Strong leadership and communication abilities
• Experience with multidisciplinary rounds and discharge planning

Responsibilities

• Participate in multidisciplinary rounds to review patient status and discharge plans
• Educate patients and families about illness impact and resources
• Assess readmissions and reasons for readmission
• Organize and facilitate care conferences
• Document discharge planning evaluations and barriers to progression
