Job summary

Piedmont Mountainside Hospital is seeking a Care Manager to coordinate patient care, oversee progression and discharge planning, and ensure timely and safe discharges for all patients in their caseload. This role supports a collaborative healthcare team within a hospital setting.

The position requires an RN license in Georgia (or multi-state licensure) and at least 2 years of acute/post-acute experience, with InterQual/Milliman experience preferred. Master’s degree is an advantage.

Qualifications

• Graduate of Nursing Program is required.
• 2 years of experience in an acute or post-acute setting is required.
• RN license for Georgia and/or multi-state licensure required.
• Experience with InterQual or Milliman is preferred.
• Master’s degree is preferred.

Responsibilities

• Care Manager is responsible for care coordination, progression of care, and proactive discharge planning; accountable for expediting timely and safe discharge for all patients in their caseload.
